This is a sample local government financial planning spreadhseet, put together by the School of Government at the University of North Carolina.  It incorporates a variety of useful fields to enter data into, which should work for most local governments - their rationale for their setup can be found here.  It is also editable, allowing for individuals who need more data field, or multiple years of budget projection to be able to do so easily.
