If you live and work in Palo Alto or are currently a student in Palo Alto (age 13 or older), please participate in the Project Safety Net Community Survey at: http://bit.ly/psn2016paloalto

The survey is available until June 24 and is offered in English, Spanish and Mandarin. Your responses will be shared with the Center for Disease Control and Prevention (CDC) Epi-Aid report, but your identity will be kept anonymous. The report is due out later this year. Please share this feedback opportunity with others in your community and networks.
