This Member Self Appraisal Form is designed to assist the member in providing input regarding their performance and to assist in obtaining a more comprehensive performance assessment. It is suggested that the employee respond to the questions in writing and provide to their supervisor in advance of the performance review meeting. Supervisors are encouraged to review and discuss the employee’s comments during the performance review meeting.
