This document is a template of a local non-medical pandemic influenza planning tool, which serves as a part of a local Emergency Operations Plan (EOP). This template is not intended to be a “boiler plate” document. It is intended to be used as a tool in the planning process, which should include all stake holders within the community and outside agencies and organizations. It suggests an overall format and examples of items that may or may not be applicable to your community. These items should be reviewed, and then expanded, deleted or modified as necessary to fit the needs of your community. Please customize the template by using appropriate logos or seals. Do not forget to replace city/county with your community. Items in blue serve as directions or information and should be removed from the final plan product.
