This document analyzes the prevalence rate of diabetes in San Antonio, which is at an alarming high. The city has created a multi-faceted plan which gives citizens the opportunity to choose which method of prevention is best suited for them. The program has been nominated for the 2016 Local Government Excellence Awards in the category of Community Health & Safety (populations of 50,000 and greater).
