This is a sample performance evaluation form for a City Attorney, put together using the combined expertise of the League of California Cities (a non-proft in the state of California).  It makes use of a combination of quantitative and qualitative analysis of the Attorney, although your local government will need to determine if the criteria given are sufficient for your use of this type of form.
