The U.S. Department of Health and Human Services (HHS) has announced the availability of $70 million in funding to improve the health of small communities across the country. Funds will be administered through the Community Transformation Grants Small Communities Program, which was launched in 2011 and is administered by the Centers for Disease Control (CDC). The program targets opportunities to improve health in communities with populations of up to 500,000.
CDC expects to award between 25-50 grants, with successful applicants announced in September 2012.

The letter of intent deadline is June 18, 2012 and the application deadline is July 31, 2012, 5 pm Eastern Daylight Savings Time.
