The Village of Schaumburg, Illinois has a population of 76,000, including many seniors and people with disabilities, who make up approximately 10% and 19%, respectively. The Schaumburg Residential Home Health Care Program was implemented to provide medical assistance to ensure that this portion of the population, who might otherwise be unable to remain in their own homes, received individualized care. As an administrative unit of the Division of Public Health and Nursing, it has adopted this mission in order to promote community health through the preventative health services, in-home care assistance, increased public health awareness, and establishment of community partnerships that foster physical and emotional well being. Particularly, the program renders assistance to recently discharged citizens with acute or chronic conditions who require ongoing monitoring and lack either health insurance benefits or the qualifications for other homecare services. To date, the program serves over 100 participants.
