Employee Accident/Incident Review committee and process

The City of Peoria AZ is interested in establishing a citywide accident/incident review program to enhance safety and employee accountability. I would like to get copies of other organizations' policies and procedures, whether you have a review committee and if so who serves on it, how it works, etc. Thanks for your help.
